You may schedule an appointment by calling one of our offices directly, or your family physician can schedule an appointment. Patients are asked to arrive 30 minutes early for their first appointment to complete the necessary paperwork.

8:00am-6:30pm Monday – Thursday
8:00am-4:30pm Friday

What to bring to your appointment:

  • Your written prescription
  • Nonrestrictive, comfortable clothing and footwear. (shorts are recommended for back or leg problems)
  • Insurance information/insurance card
  • List of medications with dosages
